A 4DoF couch leaves three rotations on the table — pitch, roll and yaw. SGRT sees them; without a robotic couch you can't act on them. TruPose closes that gap on bore-class linacs with three colour-coded knobs at the base of the neck and ±4.0° of range on each axis.

Where the missing three degrees actually bite
For single-isocentre / multi-metastasis treatments, residual yaw of 1° translates into geometric miss that scales linearly with the target-to-iso offset. Zeroing the SGRT-reported rotation before beam-on protects every off-iso target equally.
Fractionated SRT for cranial / base-of-skull targets benefits from inter-fraction reproducibility. The two-pin indexing bar + carbon-fibre frame ride the couch the same way every day; the three knobs zero the residual rotation per fraction.
Halcyon, Ethos and Radixact ship with translation-only couches. TruPose adds the three missing rotations without the cost, footprint or QA overhead of a HexaPOD-class robotic system.
SGRT reports the residual pitch/roll/yaw delta in real time. TruPose closes the loop: read the delta on the SGRT monitor, turn the matching colour-coded knob until it reads zero. No imaging dose.
Thermoplastic-mask frameless SRS replaces invasive head frames but historically left rotational corrections unaddressable on bore-class linacs. TruPose is the missing rotational link in a frameless workflow.
Bore-class linacs ship with translation-only couches. TruPose adds the three rotational degrees of freedom so the SGRT-reported delta is something you can act on — without a robotic couch.
Covers the residual rotational error budget documented in Dhabaan 2012 (max yaw ~1° after 6DoF correction). Four times the published worst-case residual gives margin for setup reality.
Light enough for the radiation therapist to move between fractions, rigid enough to hold setup under the head + mask + cushion mass. Radiolucent across CT, CBCT and MV imaging — no beam-attenuation surprises.

For Medical Physicist
Bore-class linacs (Halcyon, Ethos, Radixact) have 4DoF couches — translations plus a single rotation. SGRT reports the residual pitch/roll/yaw deltas. Without a robotic couch, how big does a 1° residual rotation actually look in the dose distribution for a 1.5 cm intracranial target, and where does TruPose plug into the workflow? Sourced to Dhabaan 2012 (4DoF vs 6DoF dose impact) + Liu 2015 (motion-correction tolerances).
Read this Medical PhysicistA clinic with a standard radiotherapy positioning board wants to deliver single-iso multi-met VMAT-SRS. At 1°, 7 % of targets drop below D95 95 % (Roper 2015); at 2°, only 63 % retain coverage. Three upgrade paths exist — robotic couch, head frame, or TruPose + open-face mask + SGRT. Which is right for a bore-class linac? Sourced to Roper 2015, Selvan 2019, Han 2022, Dhabaan 2012 and AAPM TG-147.
