Where the missing three degrees actually bite
Intracranial SRS — single-isocentre, multi-target
For single-isocentre / multi-metastasis treatments, residual yaw of 1° translates into geometric miss that scales linearly with the target-to-iso offset. Zeroing the SGRT-reported rotation before beam-on protects every off-iso target equally.
Head-and-neck SRT
Fractionated SRT for cranial / base-of-skull targets benefits from inter-fraction reproducibility. The two-pin indexing bar + carbon-fibre frame ride the couch the same way every day; the three knobs zero the residual rotation per fraction.
Bore-class linac without a robotic couch

Q.Why does a bore-class linac struggle with intracranial SRS accuracy?
Bore-class linacs (Varian Halcyon, Ethos, Accuray Radixact) ship with translation-only couches — three degrees of freedom (X / Y / Z) but no rotational correction (pitch, roll, yaw). Modern SGRT (surface-guided radiation therapy) systems report the residual rotational delta in real time, but on a translation-only couch there is nothing to act on that reading. A 1° residual yaw on a single-isocentre multi-metastasis SRS plan translates linearly into a geometric miss that scales with the target-to-iso offset — the off-iso target moves out from under the plan.
Q.How does TruPose close the 6DoF gap without a full robotic couch?
TruPose adds the three missing rotational degrees of freedom (pitch, roll, yaw) as a mechanical head-adjuster with ±4° per axis. The therapist reads the SGRT delta, dials the matching colour-coded knob until the value zeros, then treats. The rotational correction is applied at the head plate — no imaging dose, no robotic couch, no capital-equipment tier upgrade. Bore-class SRS becomes a 6DoF workflow.
Q.Does ±4° cover the residual rotational error that SGRT actually reports?
Dhabaan 2012 (CBCT-based 6DoF positioning study) documented a maximum yaw of around 1° after 6DoF correction on frameless cranial SRS. ±4° gives a four-times margin over the published worst-case residual — plenty of range for the setup reality across the fraction series.
