The cabinet for the highest-dose phase of theranostic therapy — patient administration

Lu-177 DOTATATE / PSMA-617 PRRT infusion

The standard 7.4 GBq Lu-177 dose is delivered over ~30 minutes via syringe pump into a slow IV drip. The β/γ hybrid variant wraps the pump and syringe — the low-Z inner absorbs the β before it reaches the lead, the lead outer catches the 113 / 208 keV γ. Published staff dose for PRRT administration is dominated by this step (Riveira-Martin 2023: 69 % of total staff dose in 19 % of procedure time). The cabinet is built for exactly that phase.

Y-90 microsphere or peptide infusion

Y-90 is essentially pure β (E_max 2.28 MeV). Raw lead in the β path generates bremsstrahlung at clinically meaningful rates. The β/γ hybrid variant is the right shielding order: low-Z inner first to absorb the β with minimal bremsstrahlung; lead second to handle anything else. Inverting the order — lead first — generates the worst-case bremsstrahlung the shield is supposed to prevent.

I-131 oral-solution decant and administration

I-131 at 364 keV is a lead-only pathway. The lead-only variant family covers the syringe-pump-driven oral-solution route used in some I-131 therapy workflows (paediatric, dysphagia, repeat fractionated dosing) in the right chassis form factors.

Routine Tc-99m / Tl-201 line infusion

At low-energy SPECT photopeaks, lead-only shielding in a compact form factor is sufficient. The compact lead-only variant fits the routine SPECT-isotope IV-line-shielding use case at lower chassis weight.

Bedside patient transport

The IV-pole-cut β/γ variant lets the cabinet ride on a standard IV pole from the radiopharmacy hot lab to the patient bedside or back to the ward — keeping the infusion shielded across corridor traverse and minimising the staff dose attributed to transport.

AERB-licensed theranostic suite

A theranostic-suite licence application has to identify the shielded equipment for each patient-administration step. The variant matrix lets the AERB licensee map per-isotope work onto a specific tier with traceable specs and ISO documentation.

For Lu-177 and Y-90 the shielding order matters as much as the total thickness. A low-Z absorber inside stops the β with minimal bremsstrahlung; high-Z (lead) outside catches the residual γ and any bremsstrahlung that did get through. Inverted order — lead first — turns the lead into a bremsstrahlung source. The β/γ hybrid variant is built in the correct order; the low-Z absorber is the inner wall, the lead is the outer wall.

Q.Why do Lu-177 PRRT, Y-90 and I-131 infusions need a shielded pump cabinet rather than just a shielded syringe?

Therapy-scale infusions run 20–90 minutes with the full patient dose held in the syringe or IV bag on the infusion pump. Across that time the pump sits next to the patient bed, the nursing staff, and any family in the room. A shielded pump cabinet contains the entire pump + dose during the infusion — shielding the patient environment for the duration of the delivery, not just for the transit. On a 7.4 GBq Lu-177 PRRT dose that is the difference between a routine infusion and a room-scale dose incident.

Q.When does the department need the β / γ hybrid variant rather than lead-only?

Lu-177, Y-90, Sr-89 and P-32 emit beta particles alongside (or instead of) gamma. Pure-lead shielding on a beta emitter produces bremsstrahlung — secondary X-rays from beta deceleration in the lead — which then irradiates the patient environment. The β / γ hybrid variant uses a low-Z β-absorber inner (Perspex / PMMA / acrylic) to stop the beta before it reaches the lead, then the lead outer wall attenuates the gamma. Correct architecture for Lu-177 PRRT, Y-90 microsphere and Y-90 antibody infusions.

Q.What is the IV-pole-cut variant for?

Some therapy protocols wheel the shielded pump on the patient IV pole from the infusion room to a recovery bay after the therapy dose is administered — the residual activity in the tubing continues to decay after the bulk dose is delivered but still needs shielding. The IV-pole-cut variant has a back-panel cutout that mounts to the IV pole so the cabinet becomes cart-mounted for patient-transport shielding.
